SIDE EFFECTS:
The most frequent side effects of ALPINOL taken at lower doses are drowsiness or light headedness, which probably reflect the normal actions of the drug. Side effects of higher dosages (those used for panic attacks) include fatigue, memory problems, speech problems, constipation, and changes in appetite with resultant changes in weight.
ALPINOL can lead to addiction (dependency), especially at high dosages over prolonged periods of time. Abrupt discontinuation of ALPINOLafter prolonged use can lead to symptoms of withdrawal such as insomnia, headaches, nausea, vomiting, light headedness, sweating, anxiety, and fatigue. Seizures can occur in more severe cases of withdrawal. Consequently, patients on ALPINOL for extended periods of time should slowly taper the medication under a doctor's supervision rather than abruptly stopping the medication.

CONTRAINDICATIONS:
  • Co-administration of Itraconazole or ketoconazole.
  • If the patient is allergic to alprazolam or to other benzodiazepines, such as chlordiazepoxide, clorazepate, diazepam, lorazepam or oxazepam.
  • If the patient has
    • asthma, emphysema, bronchitis, chronic obstructive pulmonary disorder (COPD), or other breathing problems
    • kidney or liver disease (especially alcoholic liver disease);
    • history of depression or suicidal thoughts or behavior;
    • history of drug or alcohol addiction.
  • FDA pregnancy category D. ALPINOL can cause birth defects in an unborn baby. ALPINOL can pass into breast milk and may harm a nursing baby.
  • The sedative effects of ALPINOL may last longer in older adults.
  • Accidental falls are common in elderly patients who take benzodiazepines. Use caution to avoid falling or accidental injury while you are taking ALPINOL.

DRUG INTERACTIONS: Serious interactions with certain azole antifungals (e.g., itraconazole, ketoconazole), delavirdine, indinavir, sodium oxybate.

MISSED DOSE:
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and take the medicine at your next regularly scheduled time. Do not take extra medicine to make up the missed dose.

OVERDOSE:
Seek emergency medical attention if you think you have used too much of ALPINOL. An overdose of ALPINOL can be fatal.
Symptoms of ALPINOL overdose may include extreme drowsiness, confusion, muscle weakness, loss of balance or coordination, feeling light-headed, fainting, or coma.

PRECAUTIONS:
  • Alcohol. ALPINOL can increase the effects of alcohol. ALPINOL can cause side effects that may impair thinking or reactions.
  • Avoid using other sedatives (such as cold medicine, pain medication, muscle relaxers, and medicine for seizures, depression or anxiety). They can increase some of the side effects of ALPINOL.
  • Grapefruit and grapefruit juice may interact with ALPINOL and lead to potentially dangerous effects.
